Pharmacy Residency Prerequisites

Graduate of an accredited School of Pharmacy with a PharmD and eligibility for licensure in Kentucky. All application materials MUST be submitted via PhORCAS. No application materials will be accepted via US mail, email or by submission in person.

Required application materials include:

  • Curriculum vitae (CV)
  • Letter of intent – why you are pursing this residency and the experiences you hope to gain for your personal and professional growth; maximum of 500 words
  • Office college transcript(s)
  • Three letters of recommendation

Admission Policy

Residency candidate selected must comply with UofL Health employee requirements and institutional policies and procedures. This includes drug screen and immunizations.
