Which Program is Right for Me?


Unlike other exercise and weight management programs, the Healthy Lifestyle Centers are grounded in evidence-based medicine. Our team works under the supervision of UofL physicians and includes registered nurses, masters-prepared exercise physiologists, a registered dietitian, respiratory therapist, and licensed clinical social worker. They serve as health coaches, identifying services within UofL and the community that could benefit the patient, and navigate them to ensure easy access.

Healthy Lifestyle Center Programs

The UofL Healthy Lifestyle Centers offer three unique programs to help you reach your health and wellness goals. Read more about each below to see which program may be right for you.

Cardiac Rehabilitation

If you have had any of the following events with in the last year, you may qualify for our Cardiac Rehab Program. This program is covered by most insurance. 

  • Heart Attack
  • Bypass Surgery
  • Valve Surgery
  • Heart Transplant
  • Angioplasty / Stent placement
  • Heart Failure

Pulmonary Rehabilitation

If you have been told you have the following, you may be eligible for Pulmonary Rehab, which is covered by most insurance.

  • Chronic Bronchitis (COPD)
  • Emphysema (COPD)
  • Lung Transplant (candidates and recipients)
  • Pulmonary Fibrosis
  • Alpha-1 Antitrypsin Deficiency
  • Bronchiectasis Obliterans Organizing Pneumonia (BOOP)
  • Cystic Fibrosis

Lifestyle Connect Program

If you have one or more of the following risk factors, you may consider our Lifestyle Connect Program.

  • High Cholesterol
  • High Blood Pressure
  • Diabetes
  • Pre-Diabetes
  • Overweight
  • Stress
  • Smoking
  • No Exercise
