Recovery from a brain injury is a long-term process. While most patients are discharged home after only a few weeks of inpatient rehab, ongoing services will be needed to continue the recovery process. The highly-skilled professionals at Frazier Rehab recognize how difficult it is for survivors and families to cope with the effects of brain injury. Your rehabilitation team will make recommendations to support your recovery.
Our outpatient brain injury rehabilitation services fall under two types of programs:

Individual treatment plans are developed in collaboration with the client and their support system. It is important to us that you feel involved and participate in your care. We will involve you in ongoing education and treatment planning throughout your course of therapy.
Frazier’s NeuroRehab Program is a comprehensive outpatient neuro-rehabilitation program for adolescents through adults who have sustained an acquired brain injury or other neurological injury, including:
The NeuroRehab Program is comprised of an interdiciplinary team of experts, including speech-language pathologists, occupational therapists, physical therapists, and psychologists. Our team approach ensures we address all aspects of rehabilitation - physical, cognitive, vocational, social/interpersonal, and emotional.
Individualized treatment plans are developed in collaboration with our patients, their support system, and your payer source. Through innovative and adaptive therapies in real world situations, our ultimate goal is to help patients become as independent as possible.

In addition to the services listed above, our interdisciplinary team of experts will provide you with additional recommendations and referrals that may promote your recovery. These recommendations may include but are not limited to: neuropsychological evaluation, vision specialist, specialized residential brain injury treatment program, rehab physician/physiatrist, home health therapies, skilled nursing facility placement or an adult day treatment program.
